New Patient Forms
We appreciate your filling out your new patient forms online, before your first appointment with us. It saves you time (no need to arrive early to do your forms) and gives you the opportunity to be as complete and detailed as possible. The more info we have, the more able we are to meet your specific needs.
Dental Insurance & Financial Matters
Our office is fee-for-service. We do not participate in any dental benefits plans. However, if your policy allows you to see out-of-network providers, we’re happy to help you with the documentation needed to submit your claims for reimbursement.
