Or the bargain appliance sends your electric bill through the roof. Or you discover that the low price doesn’t include half the things you actually need. That’s the trouble with deciding what something really costs. The answer depends an awful lot on what you count and where you stop counting. [A recent study in the _BDJ_](https://www.nature.com/articles/s41415-026-9797-1)offers a great example of this. For it, researchers compared the long-term costs of the two most common materials used to fill cavities: mercury amalgam and composite resin. By their calculations, amalgam came out ahead. It lasted longer, required fewer visits and less treatment time, and cost less over the lifetime of the restoration. “Except for appearance,” wrote the researchers, > all other outcomes and costs favoured the use of amalgam over composites for the direct restoration of posterior teeth for all stakeholders. And that’s fine, as far as it goes. The question is, does it go far enough? Even the question about durability isn’t as simple as it may seem at first. Composite is much more technique-sensitive than amalgam. It requires careful moisture control to get a strong, reliable bond. The adhesive system matters, as does the way it’s placed and how it’s cured, among other factors that can affect a composite filling’s success. For generations, amalgam was the standard material for filling teeth, and dental education affected that. Training in posterior composites has increased enormously through recent decades, but many dentists practicing today went through dental school when amalgam occupied a much larger share of the restorative curriculum. That matters when we’re comparing how long the two materials last. [A systematic review](https://pmc.ncbi.nlm.nih.gov/articles/PMC13131031/) published earlier this year looked specifically at the factors that affect the success of Class II composites — fillings that involve the side of a back tooth where it meets the neighboring tooth. As the researchers observed, “variability in clinical outcomes frequently reflected differences in procedural precision.” In other words, some of what looks like a difference between materials may actually reflect differences in how they’re placed. Of course, developing composite skills has a cost of its own. Dentists may need to invest in additional education and training. There may be related costs for staff training, equipment, and materials. Placing a composite properly can require more time, too. All of that becomes part of the cost of providing the treatment and ultimately may be reflected in what patients pay. But the other side of the equation matters, too. When technique falls short and a restoration fails prematurely, the patient may wind up paying for retreatment, making another trip to the dental office, and losing more time from work or other responsibilities. If recurrent decay or another problem has developed around the failing restoration, the treatment needed may be more extensive than simply replacing one filling with another. So even before we get to the mercury itself, the ledger is already more complicated than a simple comparison between restoration costs and longevity might suggest. Dental amalgam is roughly 50% mercury by weight. Despite the old claim that the mercury gets locked into the filling once amalgam hardens, mercury vapor is released with every bite and swallow. The FDA itself acknowledges that exposure and [recommends against using the material in certain high-risk groups](https://www.fda.gov/medical-devices/dental-devices/dental-amalgam-fillings), including young children and women who are pregnant, nursing, or planning on pregnancy. For some people, the risks of mercury exposure add more costs to the equation. Patients who decide they no longer want mercury in their mouths may pay out of pocket to have old amalgams safely removed and replaced. For those with mercury-related chronic health problems, the road can be longer still. Indeed, some spend years bouncing from doctor to doctor before anyone considers whether dental conditions may be playing a role. By then, the costs may include years of appointments and testing, lost work time, safe amalgam removal and replacement, and the time and expense involved in supporting the body’s detoxification afterward. None of that appears on the bill for the original filling. Nor do the environmental costs. Mercury released from dentistry doesn’t just disappear. It can enter the environment through dental wastewater and human waste, as well as through burial and cremation. Managing mercury waste, limiting releases, monitoring pollution, and dealing with contamination of air, water, and soil all carry costs of their own. This is especially relevant because the authors of the new cost study themselves acknowledge limits to what their analysis measured. Environmental costs, including those associated with cremation, as well as extended patient and provider safety costs, weren’t newly calculated. The authors drew on previous work where appropriate, but these weren’t among the original costs investigated in the study. And that’s really the point. An economic analysis can tell us a great deal about the costs it was designed to measure. It can’t tell us that costs outside those boundaries don’t exist. Sometimes they’re paid by the dentist. Sometimes they’re paid by the patient, either now or years down the road. Sometimes they land on healthcare systems, communities, and taxpayers. And sometimes they’re paid in damage to an environment that doesn’t send anyone an itemized bill. The bottom line is that there’s more to the cost of a filling than the price of placing it — or even the price of replacing it. Whatever economic advantages amalgam may offer within a particular set of calculations, those advantages aren’t enough to make it the right choice for us. Sometimes a bargain really is a bargain.
